• Treatment: sharing your intake, history and prescriptions with licensed providers and dispensing pharmacies.
• Payment: processing plan fees and verifying charges with our payment processor.
• Healthcare operations: quality review, provider oversight, care coordination and platform security.
• As required by law: responding to lawful requests from regulators, courts or public health authorities.

We will not use or disclose your PHI for marketing, sell your PHI, or share psychotherapy notes without your written authorization. You may revoke an authorization at any time in writing.

• Request corrections to information you believe is inaccurate
• Request confidential communications at an alternate address or phone number
• Request restrictions on certain uses and disclosures
• Receive an accounting of disclosures we have made
• Receive notice if your unsecured PHI is breached
• Obtain a paper copy of this notice on request

We are required by law to maintain the privacy and security of your PHI, notify you promptly if a breach compromises your information, and follow the duties and privacy practices described in this notice. We will not use or share your information other than as described here unless you tell us in writing that we may.

If you believe your privacy rights have been violated, contact support@increase.health. You may also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights. We will not retaliate against you for filing a complaint.

We may change the terms of this notice, and the changes will apply to all information we hold. The revised notice will always be available on this page.
