Telehealth Consent

Last updated: August 2026

01Purpose & scope

Increase Health facilitates access to licensed healthcare providers ("Providers") who deliver telehealth consultations through our platform. These services are limited to weight management, peptide therapy and prescription skin care. Increase Health is not a primary care provider, hospital or emergency service.

By proceeding, you ("Patient", "Member") acknowledge that you have read, understood and agree to the terms of this Telehealth Consent.

02Nature of telehealth services

  • Telehealth consultations may differ from in-person visits; the provider may not be able to perform a physical examination.
  • Technical difficulties may interrupt or prevent a consultation, in which case an alternative method of communication may be used.
  • Not all health conditions are appropriate for telehealth. Your provider determines whether your condition can be adequately assessed remotely.
  • A telehealth provider-patient relationship is not a substitute for an ongoing relationship with a primary care provider.

03Consent to receive telehealth services

  • You voluntarily consent to telehealth consultations with providers affiliated with Increase Health.
  • You confirm that you have truthfully and completely disclosed your health history, current medications, allergies and other relevant information.
  • You understand that the provider relies on your self-reported history to make clinical decisions.
  • You understand that no prescription is guaranteed and that the provider may request additional information or lab work before prescribing.

04Risks & limitations

  • The inability to perform a physical examination may limit diagnosis of certain conditions.
  • Delays in transmitting information due to technology failures could affect care.
  • Security protocols are in place, but no electronic transmission is completely without risk.
  • In rare cases the information transmitted may be insufficient for appropriate medical decision-making, and the provider may refer you to in-person care.

05State availability

Increase Health telehealth services are available only in states where our affiliated providers hold active, valid licenses, and availability of specific compounded products can vary by state. You represent that you are located in a state where services are offered at the time of your consultation. If you are unsure, contact support@increase.health before enrolling.

06Emergency disclaimer

Increase Health is not an emergency service and telehealth is not appropriate for medical emergencies. If you are experiencing chest pain, difficulty breathing, stroke symptoms, severe bleeding, a severe allergic reaction, or thoughts of harming yourself or others, call 911 immediately or go to the nearest emergency room. Do not use our platform or messaging to request emergency care.

07Provider-patient relationship

  • The relationship is limited to the specific purpose of the treatment program you enroll in.
  • It does not constitute an ongoing primary care relationship.
  • It does not create a duty to provide future care beyond the scope of your active plan.
  • It may be terminated by the provider at any time if clinically appropriate or if you violate the Terms of Service.

08Data privacy & security

All health information you provide is handled in accordance with our Privacy Policy and applicable HIPAA regulations. Increase Health uses HIPAA-compliant technology to store and transmit your protected health information. Consultations are not recorded without your explicit consent, and third parties with access to PHI are bound by Business Associate Agreements.

09Withdrawal of consent

You may withdraw this consent at any time by contacting support@increase.health or by cancelling your plan. Withdrawal does not affect the lawfulness of services already rendered.

10Limitation of liability

To the fullest extent permitted by law, Increase Health, its officers, directors, employees, affiliated providers and technology partners are not liable for indirect, incidental, special or consequential damages arising from telehealth services, including outcomes related to delayed diagnosis, technical failure, or decisions made in reliance on incomplete information provided by the patient.

11Acknowledgement

  • I consent to receive telehealth medical services through Increase Health.
  • I understand my evaluation is conducted remotely and that no in-person physical examination will be performed.
  • I understand prescriptions are issued only if a licensed provider approves treatment based on their medical judgment.
  • I understand that approved treatments are fulfilled by a licensed U.S. pharmacy and shipped directly to me.
  • I understand that follow-up check-ins are required before refills may be approved.
